Chickasaw Nation Tribal Health
Tribal Health helps citizens navigate healthcare programs, determine eligibility, and find providers. The program assists with medical expenses when other resources are exhausted, covering unmet medical services, dental services, and durable medical equipment up to $5,000 annually.
Who's eligible
Tribal Citizens Only
Chickasaw citizen with a Chickasaw Nation citizenship card. Service must not be available through Chickasaw Nation Department of Health or IHS facilities. Must be medically necessary, not from a self-inflicted injury (including drug or alcohol related), and must have exhausted other resources such as insurance, Medicare, Medicaid, or any other responsible party.

How to apply
Online application available. Applicants must provide: copy of insurance (if applicable); treatment plan with estimate of cost, script and/or referral; if referred by Chickasaw Nation Department of Health or IHS facility, provide a copy of denial letter from IHS purchased/referred care; original invoice and explanation of benefits (EOB).
