Community Health Representative (CHR)
Provides First Americans with medically guided primary healthcare services when no other resource is available. Services include patient advocacy, liaison between clients and resources, patient education, and health promotion/disease prevention in the Chickasaw Nation’s 13-county jurisdiction.
Who's eligible
Tribal Citizens Only
Tribal citizenship required; must be homebound or disabled; must reside within the Chickasaw Nation. Required documents: CHR Client Data Form, CDIB or tribal citizenship card, and Social Security card.
