LifeRx Program
A yearlong lifestyle change initiative that helps prevent and delay type 2 diabetes onset. Participants learn nutrition, physical activity, and coping strategies through classes led by trained lifestyle coaches.
Who's eligible
Verify With Program
Age 18+ and at least one of: prediabetes diagnosis confirmed by blood test within the past 12 months; history of gestational diabetes; elevated risk level (determined after application review). No explicit tribal citizenship/CDIB criterion listed on source page.
