The Chickasaw Nation offers assistance to eligible citizens in enrolling in Medicare Part D prescription drug plans. Patient Benefit Assistants help individuals select the most suitable plan based on their needs and complete applications.

Medicare eligible by age or disability category. Required: Chickasaw citizenship card, physical address (P.O. Box not accepted), previous two years' income information, and a current comprehensive prescription medication list.
