Special Needs Assistance for Chickasaw Citizens with Disabilities (CN SNAP)
Provides annual reimbursement up to $500 for disability-related assistive technology and expenses needed for Chickasaw citizens with documented disabilities to participate in extracurricular activities.

How to apply
Submit completed special needs assistance application form, copy of applicant's Chickasaw citizenship, copy of applicant's current Individualized Education Plan (IEP) or documentation of disability from a physician, and receipts for disability-related assistive technology needed or expenses paid.
