Three-clinic outpatient health system providing primary care, preventive care, prenatal care, women’s health, diabetes management and wellness, physical therapy, laboratory, radiology, mammography, and medical transportation. Little Axe Health Center: 15951 Little Axe Dr., Norman. Shawnee Clinic: 2029 S. Gordon Cooper Dr., Shawnee. PlusCare walk-in clinic: 15702 E. Hwy 9, Norman (7 days/week).

Open to all Native Americans, SoonerCare members, and insured AST employees. Native American patients should have CDIB or tribal membership documentation. Formal eligibility page is under construction — contact clinic to confirm current requirements before first visit.
