Spirit Within Project — Diabetes & Wellness Program
DIHFS’s flagship health and wellness program funded in part by the SAMHSA Special Diabetes Program for Indians (SDPI). Integrates traditional knowledge and practices with evidence-based diabetes prevention and management. Pillars: diabetes education and shared medical appointments; nutrition counseling with a registered dietitian (traditional foods guidance, disease-focused “food is medicine” meal planning); physical activity coaching and group fitness classes; and community wellness events. Fork n’Road is the featured 8-week weight management program combining nutrition education and exercise, followed by Crossroads for ongoing self-management. Program is accredited by the ADCES Diabetes Education Accreditation Program (DEAP).
