Choctaw Health Center — Diabetes Management and Prevention Clinic
Dedicated diabetes management and prevention services including personalized care plans, foot and eye exams, lab work, glucose monitoring, diabetes education, nutrition assessment, and immunizations (flu, pneumococcal, hepatitis B, shingles, Tdap). Walk-ins accepted. Hours: Mon–Fri 8:00am–4:30pm (closed noon–1pm).

IHS-Eligible
Enrolled MBCI members and other IHS-eligible federally recognized tribal members with confirmed or suspected diabetes or referred for pre-diabetic assessment.
