Sac and Fox Nation RAP Health Benefits (Dental, Vision, Hearing Aid, Dentures)
Revenue Allocation Plan (RAP) supplemental health benefits for enrolled Sac and Fox Nation tribal members: Dental, Orthodontic, Dentures, Vision, and Hearing Aid programs. Applicants must first obtain a referral through Black Hawk Health Center. Each program is a one-time benefit. Funded through tribal gaming revenues.

Tribal Citizens Only Geographic
Enrolled Sac and Fox Nation tribal member living within the Sac and Fox Nation jurisdiction. Requires a prior referral appointment from Black Hawk Health Center at (918) 968-9531. Each RAP health program (Dental, Vision, Dentures, Hearing Aid) may only be used once per member.
