Any CDIB Holder
CDIB holders from any federally recognized tribe living anywhere. Eligible natural, adopted, step, and foster children under 19; pregnant individuals carrying eligible children; CN healthcare employees and dependents; active-duty non-Indian Armed Services members.

Four facilities: Durant Regional Medical Center, Hugo Health Clinic, Idabel Health Clinic, and Choctaw Nation Health Care Center (Talihina). Service area covers nine OK counties: Atoka, Choctaw, Haskell, Latimer, LeFlore, McCurtain, Pittsburg, Pushmataha, Bryan.
