Applicant must be an enrolled member of the Iowa Tribe of Oklahoma. One application covers all Tribal Assistance Programs. Forms must be complete and legible with all required documents; incomplete applications are returned for correction. Receipts must be dated January 1 through December 31 of the current year and show the vendor name. If the tribal member is not named on the lease, statement, or bill, a written statement must accompany the request confirming the member resides there. All vendors paid by the tribe must have a W-9 on file, and a request may be held pending receipt of a new vendor's W-9. The published benefit schedule lists the Education Incentive at $200.00 for an 8th grade graduate, $300.00 for a 12th grade graduate, $300.00 for GED completion, and $300.00 for vo-tech or technical trade certification completion.
