Applicant must be an enrolled member of the Iowa Tribe of Oklahoma. One application covers all Tribal Assistance Programs. Forms must be complete and legible with all required documents; incomplete applications are returned for correction. Receipts must be dated January 1 through December 31 of the current year and show the vendor name. If the tribal member is not named on the lease, statement, or bill, a written statement must accompany the request confirming the member resides there. All vendors paid by the tribe must have a W-9 on file, and a request may be held pending receipt of a new vendor's W-9. The published benefit schedule lists the Higher Education Incentive/College at $92 multiplied by credit hours for graduate level, $70 per credit hour for a 3.5 GPA or higher, $65 per credit hour for a 2.5 to 3.49 GPA, and $60 per credit hour for a 2.0 to 2.49 GPA.
