Community Health Representative Program (CHR)
IHS-funded community health workers who provide home visits, monthly health screenings, medication setup and adherence support, wound checks and dressing changes, case management, health education, and transportation to medical appointments. CHRs work closely with the Miami Area IHS Clinic.
