Community Health Representative (CHR) / Healthy Living Program
Community Health Representatives help people reach health care: health education, screening, case management, patient monitoring, vital signs and foot care for people with a diagnosed illness, home delivery of medications and supplies, help with equipment such as wheelchairs, walkers and shower chairs, and connections to Pelivan transportation. The Healthy Living exercise program offers power-assisted exercise chairs and exercise machines.
