Comprehensive diabetes care delivered by certified diabetes educators and registered dietitians: endocrinology, education programs, pharmacy services, dietary counseling, retinal screenings, fitness coaching, and case management. Also includes the CDC National Diabetes Prevention Program (DPP), a lifestyle change program to help participants prevent type 2 diabetes.
