CPN Health Services determines eligibility from the documents submitted with the new patient packet. Native American adults must provide a "CDIB or Tribal enrollment card" plus a Social Security card, driver's license or state ID, and health insurance card. Non-Native spouses of CPN tribal members qualify with the spouse's CPN enrollment card and a marriage license; CPN employees and their adult dependents qualify with an employee ID.
