CPN Purchased/Referred Care (PRC) — Members of Other Federally Recognized Tribes

Supplemental funding to pay for emergency and specialty medical services that CPN Health Services clinics cannot directly provide, for enrolled members of federally recognized tribes other than CPN who live inside the CPN PRC catchment area. Covers authorized referrals to outside specialists and hospitals. Authorized office visit referrals are generally limited to 3 appointments per referral.

"Native Non-CPN member (member of other federally recognized tribe)- must live within the CPN PRC catchment area- south of the North Canadian River, north of the Canadian River, East of Indian Meridian Road, and west of the Pottawatomie County/Seminole County line." Current proof of residency and a referral from a CPNHS primary care provider are required, and evidence must exist that alternate resources such as Medicaid have been applied for. "Patients who do not live in the CPN service area will have all referrals forwarded to their service area to be reviewed and processed."

Become an established CPNHS patient using the new patient packet, then request a referral from your CPNHS primary care provider. Contact CPN Purchased/Referred Care at (405) 964-4123 (1900 S. Gordon Cooper Dr., Shawnee, OK 74801) with current proof of residency. Notify referral clerks before each outside appointment; for emergency room visits, notify PRC within 72 hours.
