Community Health Representatives (CHR) Program
The CHR Program provides outreach, community education, follow-up, and supportive services, and participates with IHS in delivering health and medical care. CHRs are based in Poplar, Wolf Point, Brockton, Fort Kipp and Frazer, and several also serve as CHR/Dialysis transport drivers.

The program states it serves "the IHS Service Unit population on, or near the Fort Peck Indian Reservation." IHS service unit population means members of federally recognized tribes eligible for IHS care in the Fort Peck area. The program does not publish a separate application form.
