Kiowa Community Health Representative (CHR) Transportation
Community Health Representatives are “transporters of last resort.” They transport patients to Indian Health Service facilities and to health centers referred by IHS, and can pick up medication prescriptions from the IHS pharmacy when a CHR is already transporting to that facility. CHRs also provide health education, promote disease prevention, and act as a liaison between IHS and Indian people.
Who's eligible
Verify With Program
The department page describes the service and its scheduling rules but never states who qualifies — it names no enrollment, CDIB, or income requirement. It does say CHRs transport patients to IHS facilities and health centers referred by IHS, and describes CHRs as a liaison between IHS and Indian people. Call 405-901-9236 to confirm eligibility.
Where it's offered
Regional
The page publishes no service boundary. It names the destinations CHRs drive to and the notice each requires: transports to Lawton, Anadarko, or Clinton require three days' notice and transports to Oklahoma City require five; rides from the town of Carnegie to the Carnegie Indian Clinic are on demand when a CHR is available. The office is in Carnegie, Oklahoma.
How to apply
Call the CHR office at 405-901-9236 or email kiowachr@carnegienet.net to schedule. CHRs do not transport on demand: allow three days for Lawton, Anadarko, or Clinton and five days for Oklahoma City. Transports to and from health facilities require family support in the form of a family member assisting, and a CHR will not leave a client at a facility unless the client is being admitted and is under a doctor's care. Mailing address: PO Box 369, Carnegie, OK 73015.
