Members can complete the Death Benefit Disbursement Designation Form and submit it to the Member Services Manager to keep on file; the most recently dated form is honored. Claims must be submitted within twelve months of the date of death. Without a form on file, the policy pays first to the funeral home (with the death certificate and a valid invoice for unpaid services), then to whoever paid the funeral expenses, then to family in the order the policy lists.
