Who qualifies

IHS-Eligible
Geographic

Open to individuals residing in the tribal Purchased/Referred Care area — on tribal jurisdiction, trust land, and the counties that border the tribal area. Also covered: students temporarily absent from the area, travelers temporarily absent for travel or employment, children in foster care, and other Native Americans who are in the area. Individuals must apply for alternate resources first, including Medicare A and B, state Medicaid, state health programs, and private insurance; PRC funds are considered the last resort for payment. Every patient receiving services at the White Eagle Health Center must also have an active chart and Pawnee Benefit Package Program (PBPP) file at Pawnee. A CDIB, photo ID, any insurance, and a current utility bill proving residency are needed for the Pawnee chart/PBPP application; if the bill is not in your name, page 2 of the PBPP application must be notarized, and a notary is available on site.

How to apply

PhoneIn-Person

Call the White Eagle Health Center at (580) 765-2501, or PBPP enrollment at (918) 762-6507 for questions about your Pawnee chart. Once a referral is approved you have 30 days to schedule and attend the appointment, and it is your responsibility to schedule or reschedule — a missed appointment not rescheduled within 30 days closes the referral. If your Pawnee chart or PBPP is inactive you have 14 days to respond to the notice or the referral is closed. Keep your contact information and address current, and update your chart yearly.

Where it's available

The program describes its area as "tribal jurisdiction, trust land, and the counties that border the tribal area" without naming those counties, so this entry is not attached to a structured service area — confirm coverage with the clinic.
